The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
If you are a new patient to our office, please visit our patient forms page to download our Health History Questionnaire. Please print it, fill it out at home, and bring it to your first appointment. This will allow us to welcome you and promptly attend to your dental needs. If you are unable to print them or have not received them by mail, kindly arrive 10 minutes before your appointment time to fill them out. Thank you and please contact our office if you have any questions at all.
For those without dental insurance, payment is expected at each office visit, as services are provided. As a convenience, Visa, MasterCard and Discover are also accepted. In special cases, requiring extensive treatment, specific payment arrangements may be made with a front desk associate.
Insurance payments are accepted from any dental indemnity plan or PPO plan that allows you to go to any provider of your choice. To better serve our patients, we will electronically submit all dental claims to your insurance carrier.
Please remember that most insurance plans do not attempt to cover all dental services and cost. Some pay fixed allowances for each procedure, and others pay a percentage of the allowed charge. Regardless of the insurance plan, the patient is primarily responsible for any account balance.
