If you are a new patient to our office, the attached file contains our health history forms. Please print it, fill it out at home, and bring it to your first appointment. This will allow us to welcome you and promptly attend to your dental needs. If you are unable to print them or have not received them by mail, kindly arrive 10 minutes before your appointment time to fill them out.
